Collect From You

When you visit, request information or appointments on www.wdcphysiotherapy.co.uk or when booking appointments or classes or making enquiries over the phone you may be asked to provide certain information about yourself, including your name, contact details – if making a telephone payment – credit card details. (Once the telephone transaction has gone through we cross shred your card details).

For direct debit payments WDC Physiotherapy & Sports Injury Clinic does not hold your card details. We use a third party specialist secure card processing provider.

Some of the information you provide to us may be considered sensitive personal data as defined by the Data Protection Act (“DPA”) 1998. You are entitled to see the information held about you. Upon request we will provide you with information about whether we hold any of your personal information. In Clinic Forms

When you visit our clinic you will be asked to fill out standardised registration forms. This form will collect the following details:

  • Address
  • Date of Birth
  • Telephone Number
  • Email Address
  • GP Details
  • Private Medical Insurance (PMI) details (if applicable)

The purpose of this information is to allow your therapist to ensure they manage your case with the relevant due care and consideration for your requirements and your plan of care. The PMI details are used to bill your insurer as appropriate or to communicate directly with them if appropriate. None of your information is passed to a third party unless under your explicit consent to release details.

Your PMI may ask for reports of your appointments but this will only occur with your consent.

You may request us to write a letter to a consultant, GP, school, employer or another person of interest to you. In doing so we will use your name, date of birth and relevant medical details in that letter. You will also receive a copy of that letter should you so choose.

Notification Of Privacy Policy Changes

We may update our privacy policy to reflect changes to our information practices. If we make any material changes, we will notify you or our client by email (sent to the email address specified in your account) or by means of a notice on our website prior to the change becoming effective.
